A cup of coffee on an empty stomach is unlikely to cause difficulties for most individuals. Still, it can cause serious health complications for others owing to its high caffeine content.

According to the Washington Post, Kim Barrett, a professor of physiology and membrane biology at the University of California at Davis School of Medicine and a member of the American Gastroenterological Association’s governing board, stated that a cup of morning coffee on an empty stomach may aggravate symptoms for those with sensitive stomachs. This includes those who are prone to specific gastrointestinal diseases, such as gastroesophageal reflux, those who have already damaged their stomach lining, and those who have stomach erosion or ulcers.

Because cortisol, the stress hormone that causes the fight-or-flight response, is normally at its highest in the morning, some people may find that a morning coffee bump exacerbates its effects.

According to Courtney Delpra, a registered dietitian at the Cleveland Clinic, cortisol can impact blood sugar levels, therefore persons with diabetes who drink coffee may want to balance their blood sugar levels by having a breakfast meal that includes both protein and carbohydrates.

Although research has shown that coffee can be acidic and increase stomach acid production, most people will not have a problem with it. A study of 8,000 healthy people found no substantial relationship between coffee intake and any of the four major upper gastrointestinal issues, including reflux and ulcers.
